In order to receive banking services in accordance with the Law of the Kyrgyz Republic "On personal information" and the Law of the Kyrgyz Republic "On banks and banking activities", I give my consent to INKOM Microfinance Company CJSC (hereinafter referred to as MFC) to:

• collect and record personal data: last name, first name, patronymic; date of birth; citizenship; nationality; gender; type and number of identity document, date and issuing authority; TIN; actual residential address; registered residential address; information from the Social Fund of the Kyrgyz Republic; information on the availability of movable/immovable property; information on the presence/absence of tax and insurance arrears
• contributions and current patents of the taxpayer (hereinafter referred to as personal data);
• processing of personal data using (but not limited to): storage, recording on electronic media, marking, mixed processing of personal data (automated/non-automated) and their systematization;
• updating, blocking, unblocking and destruction of personal data;
• obtaining information about me from authorized government agencies and organizations in the field of implementing state fiscal policy, registration of the population and acts of civil status, vehicles, real estate;
• receiving and providing information from/to credit bureaus that form credit histories and exchange them between financial and credit organizations and other legal entities, in order to reduce credit risks, prevent over-indebtedness and simplify the procedure for reviewing and providing credit;

I am aware that:

• consent to the processing of personal data is valid from the date of consent until full fulfillment of my obligations to the MFC. Upon expiration of the storage period and achievement of the purposes of collecting personal data, they are subject to destruction within two weeks. This consent may be revoked by me early by sending a corresponding written notice to the MFC, subject to full fulfillment
• of current obligations to the MFC. In case of revocation of this consent, the processing of personal data in whole or in part may be continued in accordance with Articles 5 and 15 of the Law of the Kyrgyz Republic "On Personal Information";
• The MFC carries out operations on the processing of personal data and determines the terms of provision of banking services, in accordance with the current tariffs. In this case, the MFC has the right to refuse to provide me with banking services at its own discretion;
• The MFC will ensure confidentiality with respect to my personal data, except for cases when information constituting a banking secret must be provided to judicial authorities, a financial intelligence agency, an authorized tax authority, credit bureaus, heirs (legal successors) on the grounds and in the manner established by the Law of the Kyrgyz Republic "On Banks and Banking Activities".

I, expressing consent to the processing of personal data, confirm that:

• my personal data are reliable;
• Statement-consent to the processing of personal data in the event of the provision of personal data to authorized government agencies and third parties, the MFC has the right to disclose information about me to the required extent.
• Also, I hereby acknowledge and confirm that this consent is considered given by me to third parties and also third parties have the right to process my personal data on the basis of this Statement of Consent.
